As conditions continue to change, sometimes daily, we are committed to protecting our frontline Direct Support Professionals, as well as, all of the people we serve with intellectual / developmental disabilities.

Here is a link to our most current COVID-19 Protocol . This document will be modified, as needed, so we are able to stay current with changes in CDC or Indiana State Board of Health regulations.

The National Center on Advancing Person-Centered Practices and Systems announced the availability of a “Health Care Person-Centered Profile” to assist people with disabilities, older adults, and others to communicate their needs and preferences with hospital and other health care staff.

The tool has two pages: a Health Care Information sheet for capturing brief and vital information about the person’s health status and a Health Care Person-Centered Profile for describing who the person is, what is most important to the person, and how best to provide support—vital information that can help medical staff provide more tailored and person-centered care.
